📱 How to Select a Form Template in the Mobile App

Accessing and completing form templates in the Take5™ Connect mobile app is streamlined for ease of use. Follow this guide to select and fill out the appropriate form templates assigned by your account holder.


Step 1: ➕ Initiate Form Creation

  1. Open the Take5™ Connect mobile app.
  2. Tap the orange plus (+) button located at the bottom right corner of any page within the app.
  3. From the Create/Report menu that appears, select Select Form Template.


Step 2: 🏗️ Choose Site

  1. The app will default to the Site you’re currently signed into.
  2. To change the Site:
    • Tap the Site selector at the top of the page.
    • Scroll or search to find and select the desired Site.


Step 3: 📄 Select Form Template

  1. Tap the Select a Template box.
  2. A list of available templates created by your account holder will appear.
  3. Browse or search for the appropriate template.
  4. Tap on the desired template to select it.


Step 4: 🖊️ Create and Open Form

  1. Once the template is selected and the correct Project/Site is confirmed, tap Create & Open Form at the bottom of the screen.
  2. A confirmation dialog will appear.
  3. Tap Yes to proceed to the form.


Step 5: 📝 Complete and Submit Form

  1. Fill out the form as required.
  2. Compulsory fields are marked with an asterisk (*). Ensure all mandatory fields are completed.
  3. To save your progress and return later, tap Save Progress.
  4. Once all information is entered, tap Submit & Close to finalize the form.
  5. Upon submission, your account holder will be notified and can review your responses.
