πŸ“± How to Complete Forms via the Mobile App

Efficiently manage and complete forms assigned to you directly through the Take5β„’ Connect mobile app. Follow this guide to ensure proper completion and submission of your forms.


Step 1: πŸ“¬ Receive Notification

Upon assignment, you’ll receive an email notification and the form will appear on your Home Screen within the app.


Step 2: πŸ“‚ Access Your Forms

  1. Open the Take5β„’ Connect mobile app.
  2. On the Home Screen, open the hamburger menu and locate and tap on the My Forms box to access a list of all your assigned forms.

Step 3: πŸ” Locate and Open the Form

  1. In the My Forms list, look for forms with the status Assigned or In Progress.
  2. Tap on the desired form to open and begin completing it.

Step 4: πŸ“ Complete the Form

  1. Fill in the required information or select options as per the form’s fields.
  2. Mandatory fields are marked with an asterisk (*). Ensure all required fields are completed.

Step 6: βœ… Submit the Form

  1. Once all information is entered, tap Save & Close to submit the form.
  2. Your managers will be notified of the submission and can review your responses on their end.
