🚨 How to Create an Incident

In Take5 Connect™️, managing incidents is crucial for workplace safety. This guide will walk you through creating and managing incidents using the web portal. 👇


Step 1: 🔑 Access the Incidents Section

  1. Log into Take5 Connect™️ on the web portal.
  2. In the left-hand menu, click on Incidents to view all recorded incidents.


Step 2: 📋 Add a New Incident

To add a new incident:

  1. Click Add Incident at the top of the page.
  2. The Incident Form Editor will appear.


Step 3: ✏️ Fill in Incident Details

In the form editor, fill in the following details:

  • Date & Time: Select the date and time of the incident.
  • Summary of Incident: Briefly describe the incident.
  • Treatment Required: Choose from the dropdown menu.
  • Type of Incident: Select the type (e.g., Near Miss, Property Damage, Illness).
  • Involved Individuals: Specify who was involved.
  • Drug & Alcohol Testing Required: Select Yes/No.
  • WorkSafe Notification Required: Select Yes/No.
  • Investigation Required: Specify if an investigation is necessary.
  • Lost Time Injury: Select Yes/No.
  • Causal Analysis Findings: Provide the findings.


Step 4: ⚙️ Select Sites and Affected Users

  1. Click Select Sites to associate the incident with specific sites.
  2. Tick the relevant sites and click Select & Close.
  3. Click Affected Users, select the users affected, and then click Select.


Step 5: 📂 Add Files and Save

  1. If necessary, upload relevant files by clicking Browse Files.
  2. Once all details are filled, click Save to record the incident.


Step 6: 📑 View and Download the Incident

Once saved, the incident will appear in your Incidents View. You can download it as a PDF if required.

If you encounter issues:

  • Ensure all required fields are filled in before saving.
  • Double-check that the correct sites and users are associated with the incident.
