📋 Exporting Forms

Take5™ Connect allows you to easily export form templates and assigned forms to PDF or CSV formats. Whether you need to generate hard copies or store form data in CSV format, this guide will help you export forms quickly and effectively. 👇


Step 1: 🔑 Access the Forms Menu


1. Navigate to the Forms tab from the left-hand menu in Take5™ Connect.


Step 2: 📄 Export Form Templates


1. To export form templates, click on Form Templates from the Forms menu.
2. Select the template you want to export, then click the List Menu at the top of the screen. From here, you can export the form as:

  • PDF (for hard copies),
  • Form Structure (to import into another Take5™ Connect account).


Step 3: 📝 Export Assigned Forms


1. For forms that have been assigned, go to Assigned Forms.
2. Select the form you want to export, click the List Menu, and choose one of the following export options:

  • Export as PDF (empty or submitted forms),
  • Export as XLSX (to export form data),
  • Export Form Structure (to use in another template).

Submitted forms include all responses, while empty forms do not contain any answers.


Step 4: ⚙️ Customize Fields for Export


1. When you are creating your form, you can decide if you would like certain fields to be included or excluded in exported files, by selecting or deselecting the toggle option ‘Include in CSV and PDF exports’


❓ Troubleshooting


If you encounter issues with the layout in the exported PDF or CSV:

  • Adjust the form in the form builder to tweak the layout.
  • Ensure unnecessary fields are excluded from the export.
  • Review the export format options for checkboxes, radio options, and other fields.
