Assigning forms in Take5β„’ Connect allows you to streamline how forms are distributed to users. You can assign forms based on specific users, contractors, sites, or job titles. This guide will walk you through the process of assigning forms effectively. πŸ‘‡


Step 1: πŸ”‘ Access the Form Settings


1. To start assigning a form, navigate to the Form Settings page for the form you wish to assign. From here, you can configure the assignment settings.


Step 2: πŸ“‘ Assign Forms to the Connected Network


1. You can auto-assign a form to everyone in your connected network. To do so, tick the option to auto-assign within the Form Settings page, if this doesn’t initially appear as an option you may need to deselect the setting that says “This is a template form: Yes”.
2. However, you can filter who receives the form using the following criteria:

  • Sites: If selected, employees from the selected sites and associated contractors will be included.
  • Job Titles: Filters the user list based on specific job titles.
  • Industries: Filters users based on the industry of their employer.


Step 3: πŸ‘· Assign Forms to Employees


1. You can also auto-assign a form to employees.

  • Note that currently, this is not filtered by Job Titles or Industries. All employees from the account will receive the form once this option is selected.


Step 4: πŸ—οΈ Selecting Contractors


To assign the form to specific contractors, you can filter by the following:

  • Contractor Companies: Select which contractor accounts should receive the form.
  • Industries: Narrow down contractors by their industry.
  • Job Titles: Filter contractors by job titles.

This selection does not collect users beyond the first level of contractors, only those directly associated with the selected contractors.


Step 5: πŸ§‘β€πŸ’Ό Selecting Specific Users


To assign a form to specific users, select users directly from the filter list.
You can filter by:

  • Job Titles: Narrow the list based on the user’s job title.
  • Users: Select specific individuals to receive the form.

Currently, this selection is not filtered by Industries.


Step 6: 🌐 Automatically Assigning Forms on Site Entry


You can also choose to auto-assign the form when users enter selected sites. This option can be filtered by:

  • Job Titles: Assign the form only to users with specific job titles.
  • Industries: Assign the form based on the user’s employer’s industry.

The form will be assigned to the user when they sign in to the site. For Site Inductions, a 15-minute delayed notification will follow up with users.


Step 7: πŸ’Ύ Save Your Assignments


Once your assignments are set, click Save to apply the form assignments to the selected users, contractors, or sites.

If the form isn’t being assigned:

  • Ensure that the selected criteria match the users’ details.
  • Double-check that sites have been manually assigned if required.
  • Verify that Job Titles and Industries are properly configured.
