⚠️ Verifying Reported Hazards/Risks from the Mobile App (Site Managers)

Site managers can verify reported hazards and risks directly from the Take5 Connect™️ mobile app while onsite. This guide will walk you through how to verify reported hazards/risks in the mobile app. 👇


Step 1: 🔑 Access the Hazards Tab

  1. Open the Take5 Connect™️ mobile app and sign in.
  2. Go to the Sites bar in the bottom menu and select the Site where the hazard or risk has been reported.
  3. Click on the Hazards tab located at the top left of the screen to view the list of hazards/risks for the site.


Step 2: 📋 View Unverified Hazards/Risks

Any newly reported hazards/risks will appear as Unverified at the top of the hazard list and show that it Needs Verification.

  • Site Managers can view and manage all Unverified Hazards/Risks from this screen.


Step 3: ✏️ Edit the Hazard/Risk Details

  1. Click on an Unverified Hazard/Risk to open more details.
  2. You’ll see information such as controls, risk assessments, and descriptions.
  3. If necessary, click Edit in the upper-right corner to make changes to the hazard/risk details.


Step 4: ⚙️ Change the Hazard Status

  1. After reviewing or editing the hazard/risk, locate the Status dropdown menu.
  2. Change the status from Unverified to Verified.
  3. Scroll down and click Update to save the changes.


Step 5: 📂 Confirm the Update

  1. Once the hazard/risk is verified, the status will reflect as Verified for all users accessing the Hazards screen for that site.


❓ Troubleshooting

If you encounter issues:

  • Ensure that you have the proper permissions as a Site Manager to verify hazards/risks.
  • Confirm that the hazard status has been correctly updated to Verified.
