πŸ“‹ Approving, Rejecting Form Responses, and Viewing Audit Logs

Managing form submissions and keeping track of approval statuses is essential in Take5 Connectℒ️. This guide will show you how to approve or reject forms and view the audit logs for submitted forms. πŸ‘‡


Step 1: πŸ”‘ Access the Assigned Forms Section

  1. To begin, navigate to the Assigned Forms section in the left-hand menu under the Forms dropdown.
    Here, you can view all submitted forms that need approval.


Step 2: πŸ“‹ Approve a Form

To approve a submitted form:

  1. Click View Responses for the specific form you want to approve or reject.
  2. Click View Submitted Answers. A blue box will appear at the top, where you can add notes and approve the form submission.
  3. Write Notes visible to both the user and yourself, or Admin Notes that only admins can see.
  4. From the Status dropdown, select Approved or Approved: Work in Progress.
  5. Click Update to finalize the approval.


Step 3: 🚫 Reject a Form

  1. If the form requires changes, select Rejected from the Status drop down.
  2. The user will receive a notification and can amend the form and resubmit it. You can also add comments in the Notes field to guide the user.


Step 4: πŸ“‘ Viewing Audit Logs

To view the audit logs:

  1. Click View Submitted Answers for the form.
  2. Within the blue screen, click Audit Logs to display a list of changes made to the form, including status and verification changes.
  3. You can also Export to PDF to include the audit logs in the report.

Note: Audit logs are available for forms submitted on or after 24 Oct 2023.


❓ Troubleshooting

If you encounter issues:

  • Double-check that you’ve selected the correct status before clicking Update.
  • Ensure that all notes are saved before approving or rejecting the form.
  • Verify that the form was submitted after 24 Oct 2023 to view the audit logs.
